SAMHSA Unified Performance Reporting Tool (SUPRT)

SUPRT Update

Beginning September 2025, the SAMHSA Unified performance Reporting (SUPRT) tools are expected to be utilized and replace the GPRA tool. SUPRT consists of two components:

  • SUPRT–Administrative (SUPRT-A): A brief questionnaire completed by grantees based on information available in client records.
  • SUPRT–Client or Caregiver (SUPRT-C): A brief questionnaire completed directly by the client or caregiver.

To support the transition, we will schedule an SUPRT Kick-Off/training soon, where the requirements of the forms will be reviewed. Additional resources will be posted online in the coming weeks, including, but not limited to:

SUPRT Update

Effective August 30, 2025, grantees will no longer use the Government Performance Results Act (GPRA) to collect client-level data. This means, any enrollments after August 30, 2025, should not be interviewed with GPRA. GPRA submissions with interviews dated after August 30, 2025, will not be accepted. Beginning September 1, 2025, we will transition to using SAMHSA Unified Performance Reporting Tools (SUPRT).

Please submit all outstanding GPRAs as soon as possible, so we can ensure your data is entered before we transition to the new tool. Please continue enrolling clients in your SOR-funded programs, as the new instrument will be used to collect client-level data.

SOR-funded grantees will receive further instructions via email regarding next steps.
